Job Title: Hospital at Home Nurse

An exciting opportunity has arisen for an Band 5 Nurse to be part of the new Hospital at Home team in Inverness. Our Hospital at Home program is designed to deliver comprehensive, patient-centred care, reducing the need for hospital admissions and enhancing patient outcomes.
This service will provide acute, hospital-level care to people in their homes conditions that would otherwise require acute hospital inpatient care. The delivery of Hospital at Home will enable better person-centred care experience which avoids the risks of healthcare acquired infection, and/or long stays in hospital. Local delivery of a Hospital at Home service fits with the Scottish Government drive to enhance community services and prevent hospital admissions. Funding has been secured specifically to create this service for Inverness.
The B5 nurses will be supported by Advanced Nurse Practitioners and a medical consultant Senior Decision Maker. They will work as part of the Integrated Team being supported by AHP, Social Work and Care at Home colleagues. This Team forms part of the Intermediate Care options that are available to people in Inverness.
Job Description: We are seeking compassionate and skilled Nurses to join our multidisciplinary team. In this role, you will provide acute care to patients in their homes, working closely with ANPs, doctors, specialists, and other healthcare professionals to ensure the highest standards of care.
