Are you looking to join a highly skilled multidisciplinary team who are passionate about promoting formulation driven care? An exciting opportunity has arisen to enhance the care of young people with complex emotional needs as they make the transition into Adult Mental Health Services.

The successful applicant will work to strengthen links with relevant teams under Children, Young People and Families Health Services, as well as with other partner agencies supporting young people in the Cardiff and Vale area, with a view to ensuring that their mental health care remains cohesive, effective and formulation driven during a period which is typically associated with  significant uncertainty and change for the young person and their support network.

The post will involve assisting professionals in understanding and supporting complex, high-risk individuals.  This includes supporting teams to enhance their formulation of and response to behaviours that can challenge relationships and systems, as well as to recognise when a pattern of interaction between a service user and their care network may be inadvertently re-enacting the service user's previous experiences of being cared for.

The Cynnwys team are a warm, supportive team where curiosity is encouraged and collaborative working is valued. Supervision and CPD are viewed as essential to the role and the successful applicant will receive ongoing support to develop the highly specialist skills required for the post.

Main duties of the job

To hold a small caseload of high-risk young people with complex emotional needs who may go on to attract the diagnosis of Personality Disorder and who require transition to secondary mental health services.

To work in partnership with young people, carers and involved professionals to promote a person-centred, coproduced approach to the care and treatment that is offered to individuals who, at times of intense distress, may cause themselves serious harm.

To assist colleagues within services for Children and Young People's Emotional Wellbeing and Mental Health and within Adult Mental Health, in the development of a shared formulation and plan for a young person's ongoing care as they transition between different services.

To provide a leadership and consultancy resource, providing education and support to multidisciplinary colleagues in the effective care and treatment of individuals with complex emotional needs.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
