Are you an innovative Speech and Language Therapist seeking a specialist community role with system impact? Join the Provider Improvement and Response Team (PIRT), improving outcomes for residents in care and nursing homes across Staffordshire and Stoke-on-Trent.

This permanent post is initially aligned to a 12-month project supporting care home quality improvement and intensive support. The role requires you to work closely with the multi-disciplinary teams/agencies to support with a range of complex care/issues. Care planning will not always be straightforward and will need adjusting from time to time, preventing unnecessary hospital admissions, and improving the health and wellbeing of the population.

This role also contributes to system-wide learning by identifying and communicating potential barriers that may increase the risk of hospital admission, such as unmanaged swallowing difficulties or communication breakdowns, and by supporting the CARE project Lead Nurse in evaluating and refining project objectives. Strong motivational, persuasive and negotiating skills are essential, along with flexibility, a proactive approach, and the ability to organise workload effectively and work collaboratively as part of a team. There may also be an opportunity to develop this role in line with the service.

Due to the nature of this role, you will be required to travel independently and adequately across a large geographical area.

Main duties of the job

Secondment applications are welcome, subject to manager agreement.

About the role

You will work autonomously within a multi-disciplinary team, supporting care home providers to improve safety, wellbeing and outcomes. You will undertake holistic assessments, deliver targeted interventions, build staff capability and contribute to admission avoidance.

You will also support system learning, quality improvement, and work with partners across Local Authority, primary care, acute and voluntary services.

You will receive clinical supervision via MPFT CHIST and AHP services.
