Job overview

You will be responsible to facilitate and manage the provision of high quality care to adult respiratory patients alongside the integrated consultant in respiratory medicine in a variety of settings to promote respiratory health within Imperial Healthcare College.

Primarily the role is within the community setting, however an element of secondary care work will also be involved. You will provide support and advice to doctors, nursing staff, allied health care specialists, patients and their families/carers, attending the Trust, either as inpatients or outpatients and ensure smooth transitions of care into the community setting.

Ideally you will have excellent experience and knowledge of working within a respiratory setting. You will require good organisational and interpersonal skills, needing to be able to work autonomously as well as within a team.

Main duties of the job

  • Work with the Respiratory team in assessing, planning, co-ordinating, delivering and evaluating care for patients receiving pulmonary rehabilitation, respiratory Hospital at Home (early discharge and admission avoidance schemes), home oxygen, stable disease management at home and those attending respiratory diagnostic or review clinics
  • To assess and treat patients within the speciality who may have highly complex needs and maintain accurate records.As a respiratory practitioner and a member of the multidisciplinary team, contribute to the development of the respiratory services.
  • To lead best practice in respiratory care of adult respiratory patients in a variety of settings, predominantly within community, but also secondary care. To advise on chronic disease management using clinical guidelines.
  • To support the team to provide Remote Monitoring of Patients following acute admission for respiratory illnesses.
  • To assist in the co-ordination of patient care across the patient pathway, developing the pathway across the various settings to maximise patient satisfaction and outcomes to achieve a seamless service.
  • To undertake assessments of complex respiratory patients with co-morbidities, including physical examination, and to develop treatment plans for patients using investigative and analytical skills, and to formulate individualised management and treatment plans using clinical reasoning.
