As the demand on provision of Renal services is continuously growing,  new and innovative ways of working must be sought in order to meet the requirements of excellent patient care. We believe that new innovative posts such as this, which harness the excellent work pharmacy technicians do in supporting patient care, are the way forward, and are strongly supportive of developing our technicians in new and exciting ways.

The successful post holder will work within the Renal team to provide additional pharmacy input into our dialysis and renal outpatient population.

Work closely with clinicians, nursing colleagues and other members of the MDT to promote safe and effective use of medicines.

You will have the opportunity to gain experience across all aspects of renal pharmacy, with strong encouragement and support to develop your knowledge, skills and expertise.

The role will be based at the RDH site, but the post holder may be required to work cross-site  ( at QHB and SJCH ).

Main duties of the job

This is a new role and will be constantly evolving: it is envisaged that the main duties of the role will include

  • Supporting provision of pharmacy operational elements to the renal service in conjunction with the Advanced and Specialist Renal Pharmacists.
  • Providing technical support to clinical elements of the service including regular medicines counselling, medicines reconciliation and medication reviews for our dialysis patients.
  • Helping to ensure correct and timely supplies of medications to renal outpatients.
  • Supporting Renal Pharmacists in answering queries from Pharmacy Homecare and renal patients on Homecare.
  • Counselling and registering renal transplant patients on homecare service.
  • Ensuring cost-effective use of medications within renal service and compliance with local and national guidelines.
  • Monitoring, auditing and evaluating effectiveness of the pharmacy renal service, to identify service development opportunities and working with renal team to implement these.
  • Liaising with primary care including GPs, nurses and pharmacy teams to support continuity of care between primary and secondary care.

The successful post holder will receive extensive training to undertake these new responsibilities.

If not already completed, the successful post holder will be expected to complete the NVQ level 4 Pharmacy Services or equivalent as part of their development plan
