Job summary

Main duties of the job

The nominal base for this role will be St Benedicts Hospice and Centre for Specialist Palliative Care in Sunderland, however you will be required to work across the whole of Sunderland and surrounding areas to provide care to patients in their own homes.

The hours of work are between 9am-5pm Monday to Friday, with flexibility depending on service requirements.

Good communication skills and some knowledge and experience working with patients who have life limiting illness. Experience of MDT working and managing own caseload. A desire to work within a Team who deliver high quality patient care and work with their extended team in the community. Some understanding of the current local and strategic Palliative Care Issues and recommendations to support ongoing good practice. To have an understanding of how primary care is provided to patients in the their own home.
