We’re thrilled to offer a fantastic opportunity for a Band 6 Occupational Therapist to join our forward-thinking Halton Intermediate Care and Frailty Service (HICAF). We’ve been on an inspiring journey to transform intermediate care in Halton, bringing together health and social care into one integrated, collaborative team. You’ll be part of a service that truly puts people first, working in their own homes or in discharge-to-assess units, following a “home first” approach.

You’ll have opportunity to rotate across key areas of our service, gaining diverse experience and making a meaningful impact:

• Single Point of Access
• Community Rapid Response / Frailty Team
• Reablement Team
• Intermediate Care Unit

Join our vibrant multidisciplinary team including GPs, Advanced Nurse Practitioners, Physiotherapists, Occupational Therapists, Social Workers, Therapy & Nursing Assistants, Dieticians, Care Support Workers, and Administrators. Collaboration is at the heart of everything we do.

Main duties of the job

• A qualified OT with relevant experience

• Carry out holistic therapy assessments and provide clear diagnoses
• Design and deliver personalised treatment programmes that empower patients
• Collaborate with a multidisciplinary team and liaise with external agencies to ensure seamless care
• Contribute to service improvement projects, audits, and in-service training
• Support and supervise students and junior staff, helping shape the next generation of therapists
• You’ll need to be adaptable and ready to travel across Halton to meet service needs.
