UHCW NHS Trust has a new and exciting opportunity for Occupational Therapists!!

With frailty and an ageing population being at the forefront of evolving services across the NHS, now is the time to get involved and be part of the positive impacts we can have for our patients.

We are looking for Occupational Therapists that are passionate about supporting frailty patients within an acute setting  to join our new rotation.

The rotation currently covers the Care of the Elderly and our Medicine wards, with the aim to expand into other relevant clinical inpatient areas.

Care of the Elderly covers 4 x wards,  serving our older population.  The main conditions treated include: delirium, falls,  dementia, infections.

Medicine covers 4 wards, serving a general medical caseload.  This includes endocrinology, respiratory and infectious diseases.

Within both clinical teams, the patient population are often frail and at high risk of reconditioning, therefore if you have well-rounded inpatient knowledge and an interest in frailty and preventing deconditioning, this may be the post for you!

As a Band 6 into a new role, you will be supernumerary for 2 weeks.  This will allow you to get to know the team, service and Trust and highlight any training needs.

Main duties of the job

  • To manage a caseload of patients with varying diagnosis and / or complex needs, using evidence-based practice with client centred principles, to assess, plan, implement and evaluate interventions
  • To support the learning and development of students and staff
  • To develop enhanced clinical skills and knowledge in an area of interest linked with patient cohorts (e.g. cognition, specific clinical condition, prehabilitation / acute rehabilitation)
  • To participate in 7-day working patterns as required

Detailed job description and main responsibilities

  • Be professional, legally responsible & accountable for all aspects of own work and activities, to manage clinical risk within own clinical caseload.
  • Carry a clinical caseload of highly complex patients, provide advanced therapeutic assessment & evaluation, and provide detailed written treatment plans with patient centered goals working within ward / department areas.
  • Work as an autonomous practitioner and member of the multidisciplinary team, contributing to team discussions and influencing decisions about patient care programs, taking on the role of care coordinator and/ or vocational lead where relevant.
  • Carry out treatment, using a broad variety of modalities and clinical reasoning to allow selection of the most appropriate technique evaluating, modifying, and recording all interventions and outcomes & adapting them to meet the needs of the patient.
  • Use highly developed manual treatment skills requiring coordination, sensation & dexterity.
  • Work in accordance with RCOT & HCPC rules of professional conduct and within local care pathways, policies and standards.
  • Establish and maintain effective communication networks with patients, carers, MDT, other workers and agencies across health, social care and the private sector locally and regionally.
  • Contribute to the Trust and Group clinical governance arrangements and quality agenda, including the setting and monitoring of practice standards, clinical risk’s
  • Take the lead for information management within area of responsibility to ensure     high quality up to date written and electronic (where available) records and activity data are maintained in accordance with Professional and Trust standards & produce in a timely manner.
