Job overview

The post holder will work as part of the Complex Nurse Assessor Team, to support and coordinate discharges for the service. The post holder will be based in Manchester Foundation Trust and community locations working as part of a multidisciplinary  team working in collaboration with other services to facilitate the timely referrals, support appropriate discharge from hospital and complete timely assessments with the community setting working support and interventions in the community working closely with colleagues across all sectors and Urgent Care Services, Integrated Care Boards and Local Authorities.

Main duties of the job

The post holder will be based in Manchester Foundation Trust and community locations working as part of a multidisciplinary  team working in collaboration with other services to facilitate the timely referrals, support appropriate discharge from hospital.

The post holder will have knowledge of current processes and discharge pathways for medically optimized patients awaiting transfer from hospital and community settings working within the new policy and operating model. They will provide and/or arrange appropriate support and  in the community working closely with colleagues across all

sectors and Urgent Care Services. They will work flexibly, interchanging between hospital and community working as required providing the full discharge to assess service cover. The role requires working as an integral

part of the team to provide specialist assessment skills, knowledge and management of patients in the community whilst ensure the delivery of high-quality care, and efficient use of facilities, staffing and other resources.

The post holder will provide specialist knowledge and clinical skills to ensure the delivery of high quality safe care within the service and in the wider community and across the acute/community interface. He/she will actively participate in improving the service the department/unit offers and will initiate and support future development. They will contribute

to effective budgetary and resource management, training and development .
