Are you ready to take on a new challenge? Do you have a passion for innovation in public health and digital service delivery? We are seeking an experienced and motivated Health Visitor to join our newly established Digital Health Visiting Team within Kent Community Health NHS Foundation Trust (KCHFT). This innovative team serves as the central access point for the Health Visiting Service in Kent, delivering digital and remote support to families and partner agencies in line with the Healthy Child Programme. As a Health Visitor in this service, you will play a lead and pivotal role in delivering high-quality, remote care through triage, safeguarding, and multi-agency collaboration and be confident using digital technologies. Working within a skill-mix team, you will take responsibility for delegation, supervision, and clinical oversight, ensuring every family receives timely and effective support. This is a hybrid role, predominantly home-based, with regular virtual engagement and one clinical practice day per month in the community to maintain professional competencies and support team cohesion.

Main duties of the job

Clinical Delivery:

  • Provide phone-based and digital support via Let’s Chat 0–5 SMS service, Central Advice Line, and Safeguarding Hub in line with SOPs and governance frameworks.

Triage and Case Management:

  • Assess and action all incoming referrals, enquiries, and tasks to ensure appropriate and timely care, safeguarding and adherence to Healthy Child Programme requirements.

Safeguarding:

  • Review domestic abuse notifications, attend and contribute to strategy/safeguarding meetings, and work with multi-agency partners.

Team Leadership and Delegation:

  • Supervise and delegate work to skill-mix team members, supporting a streamlined and efficient service.•Partnership Working:Collaborate closely with 12 District Health Visiting Teams to ensure effective communication and seamless service delivery across the county.

Remote and Onsite Working:

  • Participate in daily virtual check-ins, 1:1 supervision, and monthly in-person clinical practice days to maintain competence and team connection. A car driver is essential to the role
