Job overview

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

As part of a team of Clinical Pharmacists and technicians you will be responsible for providing safe and effective medicines optimisation services to patients referred into the Integrated Pharmacy Team (IPT) and agreed community hospital sites. In addition, you will provide pharmaceutical advice to a range of specialist services.

You will develop an expertise in the optimisation of medicines in frailty, working with multidisciplinary teams to ensure that we achieve the best medicines related outcomes for the patients we serve.

Main duties of the job

  • Optimising medicines choice through medication review for patients in their own home or community hospital setting
  • Ensuring safe transfer of care into community hospitals including medicines reconciliation
  • Participation in KCHFT and primary care network (PCN) multidisciplinary team patient reviews
  • Assessment of concordance and implementation of appropriate interventions
  • Providing responses to medication queries
  • Provision of legal and ethical advice as appropriate for the service provision
  • Undertaking clinical development and contributing to the development of others
  • Supporting training to staff within the pharmacy service
  • Where required undertake training to become and independent prescriber
  • Supporting and facilitating Quality Improvement (QI)
  • Liaising with key stakeholders in order to explore identified risk areas
