We are seeking enthusiastic, compassionate Social Workers who hold a strong belief in recovery for people with severe and enduring mental health problems. Our Community Mental Health Social Workers hold a caseload and undertake the role of care coordinator for those with varied complex mental health and social care needs.
We are looking for someone with an interest and understanding of the issues people with mental health needs may experience is essential for this post as is a good understanding of Care Programme Approach, Care Management and Safeguarding.
Delivering high quality community mental health care in Welwyn Garden City and Hatfield and surrounding villages, you will be based at Rosanne House.
Main duties of the job
As a ASYE Social Worker you will be expected to:
- Provide assessment and specific interventions to meet the needs of service users, their carers and families.
- Be a flexible practitioner who enjoys new challenges in meeting the developing needs of the service and our service users.
- Demonstrate the ability to work well within a team, prioritise and manage own workload and display initiative whilst maintaining a positive, professional and calm approach

Detailed job description and main responsibilities
- Be a champion of social care with an understanding of legislation and statutory guidance.
- Undertake assessments and reviews, for people who may present with a high risk, or those with significant interpersonal, social, legal and/or environmental complexities, ensuring people are assessed appropriately against social care eligibility criteria as defined by The Care Act 2014.
- Offer direct support to service users and their carers, and to colleagues, through information and advice, to ensure that those who do not meet social care eligibility are appropriately signposted and supported.
- Ensure that appropriate health and social care services are commissioned with and on behalf of service users, including the duty to offer direct payments, and that processes are in place for service users to be charged by the local authority for the social care services they receive, if applicable.
- Ensure that commissioned services allow choice to service users and carers and focus on people living in the community with appropriate support and access to universal services.
- Undertake regular risk assessments and ensure that vulnerable people are kept safe.
- Comply with both child and adult safeguarding practice in line with current policy, and take a role in adult safeguarding investigations, which may involve supporting the implementation of protection
