This post is part of a new service development which supports both the Social Work and commissioning staff within the Integrated Care Teams. The post holder will provide care and support interventions as directed by the Social Work Professional Lead and/or a professional staff member. The dual function of the post will ensure that the health and social care needs of Older People are met through an efficient, older people focused multi-professional approach.
The person appointed will be expected to work as a member of an Integrated Care Team to provide high quality care to adults and older people within the geographical area covered by the Team.
To provide direct patient care and practice with older people, carers and families.
Provision of a social care service to service users by undertaking tasks and activities delegated by professional team lead and other delegated professional staff, working within relevant legislation and Trust policies and procedures.
Undertake the arrangement and delivery of services assessed by the ICT professional practitioner/clinician as needed to support the service user/carer.
Undertake tasks which have been delegated and which may include coordinating care and support packages and respite services.
Assist ICT professional practitioner/clinicians in setting up and facilitating care reviews.
Participate in reviews of service users’ care plan.
Provide relevant statistics and information as required.
Undertake mandatory and other relevant training in relation to the care of adults as deemed appropriate.
Adhere at all times to the care plan in place for the service user.
Refer any concerns about the service user’s care and/or condition immediately to the delegated professional staff and carers.
Ensure that Health and Safety standards are maintained according to Trust policy and procedures.
Arrange the provision of domiciliary care packages to include completion of DC 1’s and written documentation to support commissioning of services as delegated by professional staff.
Assist the delegated professional staff in the organisation of service user reviews and action of review outcomes as directed.
Liaise with family members to keep them informed of any changes to services commissioned.
Monitor non-complex cases under the direction of the delegated professional staff and report any concerns.
Maintain accurate records in accordance with Trust Policy. This includes service user held care plan records and office based records.
Undertake office duty under the direction of the professional team lead and/or an appropriate professional staff member.
Sign-post service users and/or their carers to social, community and voluntary organisations/activities to meet their needs.
Liaison with relevant voluntary and community agencies in the provision of services.
Assist with the arrangement of admissions to bed-based short breaks and the organization of flexible short breaks and day care facilities under the guidance of the delegated professional staff.
Monitor on an ongoing basis domiciliary packages of care provided by the independent sector as directed by the delegated professional staff.
Support the delegated professional in the timely discharge of service users from hospital in accordance with the Trust’s Escalation Policy by responding promptly to the direction of the delegated professional staff.
To understand the impact of caring on carers and to refer for carers’ assessments as identified under the Carers and Direct Payments Act (Northern Ireland) 2002.
Maintenance of an up-to-date and detailed record of work in a form to facilitate supervision by the Professional Team Lead or an appropriate professional member of staff.
Preparation and maintenance of full accurate case records in compliance with any Subject Access Requests.
Adhere to all Trust policies and procedures and in particular those relating to adult safeguarding.
Take responsible for the continuous development of their own knowledge and skills in line with Trust and Northern Ireland Social Care Council requirements.
Maintain registration with the Northern Ireland Social Care Council.
Attend and participate at team meetings.
