The Pharmacist appointed will be a member of the pharmacy team providing Pharmaceutical Services to the Southern Health and Social Services Trust. The post holder will work as a specialist clinical pharmacist providing a comprehensive integrated medicines management service optimising the medicine management process, drug use and pharmaceutical care to patients to meet the medication-related needs of inpatients and patients treated by Adult Community Services (ACS) Hospital at Home (H@H) service (in the first instance) and where necessary CAH / DHH Older Peoples Assessment / in-reach Admissions.
Operational Delivery
To provide a high level of clinical pharmacy input as part of the multidisciplinary team to all patients within designated wards and those treated by the Hospital at Home team, to ensure maximum efficacy, safety and economic drug use to demonstrate professional accountability and to enhance patient care.
To lead an integrated medicines management service at allocated H@H site.
To practise as a generalist Independent Pharmacist Prescriber for patients in accordance with the Trusts’ Independent Prescribing Framework, relevant legislation and professional and/or policy.
To obtain comprehensive admission medication histories for inpatients and patients treated by the Hospital at Home team.
Plan and organise own work according to agreed objectives and timeframes for the designated area/pharmacy service.
To complete medicines reconciliation for inpatients on admission and patients treated by the Hospital at Home team.
To rationalise patients’ medication on admission to hospital or treated by the Hospital at Home team.
To ensure the appropriate management of patients’ own drugs.
To deliver a high quality, specialist ward and community based pharmacy service, including expert review of complex drug regimens, medicines reconciliation, drug interactions monitoring, adverse drug reaction monitoring and ensure timely and efficient drug supply.
Optimise patients’ medication on admission, during the inpatient stay and at discharge in accordance with patients’ diagnosis and assess treatment options available.
To support patient discharge planning with respect to medication, including the facilitation of timely dispensing.
To complete medicines reconciliation for patients at discharge and to ensure that timely discharge information is available to primary healthcare professionals.
Develop good working relationships with the other members of the healthcare team representing the pharmacy profession and demonstrating professional accountability.
To take steps to ensure patients and carers understand the purpose of their medicines and deal with any practical issues that may prevent the optimal use of their medicines.
To become expert and demonstrate a high level of knowledge of medicines usage within a specialist clinical area.
Negotiate and influence changes in prescribing practice to ensure that medication use is safe, effective and evidence based.
Attend virtual ward rounds and multidisciplinary meetings.
Proactively contribute to therapeutic decisions concerning patient management.
To further develop and implement the generic switching and therapeutic tendering changes at ward level.
To promote high quality cost-effective prescribing and to assist in the production of cost savings reports.
To forecast new drug developments and identify changes in prescribing practice that may impact on the drugs budget.
To develop and ensure adherence to local, regional and national evidence based prescribing practice guidelines.
To make evidence based decisions, in partnership with patients and other professionals within the specialist area.
To make clinical decisions where limited information is available.
To identify, initiate and evaluate drug usage reviews and implement changes in prescribing practice accordingly.
To reduce risk associated with medicines. This will include the identification and reporting of medicine related incidents and participation in the review of incidents.
To participate in the supply and dispensing of pharmaceutical products and the provision of patient services.
To provide expert medicines information to clinicians and healthcare professionals patients and carers and ensure that this information is utilised in a systematic and appropriate way to optimise patient care.
To be registered as an independent prescriber and prescribe for patients in accordance with the Trusts’ Independent/ Supplementary Prescribing Framework, relevant legislation and professional and/or policy.
Identify and respond to the specialist pharmaceutical information needs of patients, carers, doctors, nurses and other members of the healthcare team.
To ensure safe systems of working and compliance with written procedures.
To ensure that medicines are stored securely and appropriately at ward level in compliance with legislation and the Trust’s Medicines Code.
Communication and Information Management
Communicate effectively with staff and maintain productive working relationships amongst your staff and with others.
To ensure the maintenance of appropriate documentation and records.
Report and record all accidents, untoward incidents, sickness and absence within your area of responsibility using appropriate forms.
Deal with and report complaints in keeping with Trust policy and assist with investigations when necessary.
To develop and implement procedures and protocols relevant to the implementation and effective running of integrated medicines management.

To ensure that the needs of patients and their carers are at the core of the way that the Trust delivers services.

Supervise and support the junior pharmacists and technical staff working within your area on a daily basis.
Utilise the skill set of the pharmacy team working in your designated clinical area and delegate tasks when appropriate.
Training and Education
Participate in the training of undergraduate MPharm university students.
To contribute to and help deliver the training programme for Foundation Training Year pharmacist trainees.
Participate in the mentoring, teaching and training of pharmacists on the Foundation Programme.
Act as a supervisor to pharmacists undertaking the Independent Prescribing and/or Advanced Practice/MSc qualifications.
Undertake the role of Designated Pharmacist Practitioner (DPP) for those undertaking the Independent Prescribing qualification.
Supervise and assist in the training of Technicians, Student Technicians and Assistant Technical Officers.
Participate in the mentoring and teaching of pharmacy technicians on the Medicines Management Accredited Programme (MMAP).
To participate in pharmacy education sessions.
To contribute to the education and training of medical, nursing staff and other healthcare workers.
Complete continuing professional development (CPD) in accordance with Pharmaceutical Society of Northern Ireland requirements and apply knowledge gained to current and future work as appropriate.
To participate in pharmacist education sessions.
To contribute to the education and training of pharmacy, medical, nursing staff and other healthcare workers.
Research and Development
To collect and collate all relevant activity data for clinical service evaluation.
Maintain Good Clinical Practice training for Clinical Trials and participate in the dispensing of trial products when needed.
Undertake clinical audit and quality improvement projects in your designated clinical area.
Participate in research projects relevant to your clinical practice
Participate in the preparation of conference presentations, posters, writing reports and the writing up of publications.
