As a Service Planning Manager working within a high profile programme  team with freedom to act, the post holder will play a key role in leading the service planning requirements to deliver a new transformational model of care in north Powys. This programme will act as a flagship to the  implementation of a ‘seamless new model of care’ under the ‘A Healthier  Wales: Our Plan For Health and Social Care’, and will require the post holder to stay informed about and contribute to communities of learning about these new models of care as they develop across Wales

Main duties of the job

The post holder will require a level of freedom to act, to enable agile  decision making and actions they will be expected to interpret national  policy and guidance for health and social care. This role will operate in a  complex environment working across health, social care and  multiagency organisations to ensure the implementation of national and  local policy across directorates via the delivery of a new integrated model in health and social care in north Powys.  The post holder will be responsible for analysis of detailed, complex data sets and be able to undertake research in relation to national and local evidence base to make judgements on how to effectively deliver the new model of care at both a strategic and directorate level.

Detailed job description and main responsibilities

The role will involve working with clinicians, managers, staff, professionals, partners and other key stakeholders across health, social care and multiagencies to deliver a new model of care across north Powys.

This will include: • Undertake mapping of existing services and development of a detailed  robust case for change required as part of the business case  development.  • Leading on the development of new models of care, service plans  /specifications and pathways in conjunction with health and care  professionals, based on an agreed planning framework, national  guidance and legislation, Powys Health and Care Strategy and Area  Plan and local population needs and intelligence. • A significant amount of engagement and communication messaging.  The post holder will work with the Programme Lead and work stream leads to provide leadership and expertise in technical planning to deliver  transformational change and capital business case development of circa  £60m for north Powys.  The post holder will play a key role in capital business case development  from strategic outline business to full business case (FBC) for a proposed  new health and care facility. They will also be required to take a lead role on the development of other Local Authority and Health Board plans and  business cases to support the broader implementation of the new model of care. There will be a specific need to support local planning within the Newtown area. This will involve a high level of engagement with the community, service users and other key stakeholders to support the development of a Community Wellbeing Hub and Rural Regional Centre. The post holder will be at the forefront of a dynamic and high profile  programme. This will involve the post holder making a significant  contribution to all facets of health service technical planning.
