Job summary

The Senior Transitions Practitioner is a registered clinician who provides autonomous, highquality clinical care to young people within the CAMHS Transitions Service. The post holder is responsible for the assessment, formulation, planning, implementation and review of complex care plans for young people transitioning between inpatient and community settings.

The role includes clinical leadership, supervision of junior staff, and contribution to the strategic development of the transitions service. The Senior Transitions Practitioner works across inpatient and community environments, providing specialist expertise in transition planning, risk management and discharge coordination.

The post holder plays a key role in reducing length of stay, preventing readmission and ensuring safe, effective transitions by working collaboratively with young people, families, carers and multiagency partners, including adult mental health services.

Main duties of the job

Clinical Practice

Complete assessments, including risk and care planning, for young people with complex needs

Manage and prioritise a complex caseload, making appropriate decisions independently

Deliver evidencebased interventions (1:1 and group), adapting to need and risk

Monitor risk, respond to changes, and escalate concerns appropriately

Contribute clinical expertise in MDT meetings and care planning

Leadership & Supervision

Provide supervision to Band 4--5 staff, students, and trainees

Act as a role model, promoting good practice and reflective learning

Support daytoday running of the service and deputise for the Team Lead when needed

Contribute to managing risk, incidents, and service improvements

Service Development & Governance

Support service development and improvement work

Contribute to audits and outcome monitoring

Use evidence and guidance to inform practice and service delivery
