An exciting opportunity has arisen for a Senior Transformation Lead within the Transformation & Healthy Lives Partnership Team. This role will be instrumental in leading programmes of work spanning organisations, striving to improve care and outcomes for our population.   The post holder will work within a joint transformation team ensuring delivery of the integration and transformation agenda. This varied role includes:

  • project and programme planning, management and delivery,
  • reporting and tracking of progress working with our trust PMO
  • facilitating, and leading events with partners and stakeholders
  • identifying, mitigating, and managing risk,
  • stakeholder management and engagement and;
  • a strong focus on benefits realisation and outcomes achievement.

Main duties of the job

  • Lead on Trust wide and HLP transformation programmes as per the annual plan or strategic 5-year plan.
  • Define service transformation, improvement and greater collaborative working opportunities in line with the Trust’s strategy to develop the annual transformation plan.
  • Support the developed strategic and operational direction of all Programme workstreams, and support the Programme Director, Managers and Workstream leads to engage, develop and deliver integrated transformation plans.
  • Work at the interface of care and service delivery (Divisions and Directorates and Primary and Community Services) to develop clinically led pathways and approaches to care that move the focus of services and provision from the acute hospital to community and people’s homes.
  • Develop and lead/support high performing delivery team(s), acknowledging that this leadership will be in partnership with Divisions and Directorates, and service partners.
  • Contribute to system and LCP transformation priorities.
  • Lead and direct any organisation-wide programme management approach to deliver the transformational change agenda in support of the Trust’s vision and strategic priorities.
  • Provide assurance and governance on all aspects of the transformation and improvement agenda.
  • Education and skills training for transformation i.e. case for change, logic models.
  • Development of dashboard to track through implementation the transformational outcomes.

Detailed job description and main responsibilities

  • Communication and stakeholder management - ensuring that relationships are developed and maintained, thus enabling productive, two-way communication with all key stakeholders.
  • Build strong working relationships with and between Clinical teams across the Trust and enable Community, Primary and Secondary care to create a one system approach to end to end care.
  • To lead, manage and maintain credibility with all key players within the transformation projects and programmes, fostering a culture of collaboration for the delivery of equitable, high-quality care.
  • Delivery of highly complex, sensitive, potentially contentious and technical programme information relating to transformational service changes and progress to a wide range of audiences internally and across the wider system.
  • Line management responsibility within team as required or lead in partnership.
  • Co-ordination of projects to develop, deliver and implement the Integration plan.
  • Identifying, initiating and redefining projects within the programme when necessary, interpreting national guidance where appropriate.
  • Through coaching and mentoring, establish and build transformation management capability across the system with an ethos of sharing information and learning.
  • Benefit management – following detailed analysis of a range of options, defining, quantifying, measuring and monitoring benefits, sharing information to support informed engagement to projects.
  • Identifying, escalating and managing risks and issues
  • Compiling business plans, developing strategy and ensuring implementation supporting achievement of performance targets and strategic objectives.
  • Develops policies and processes for programme of work where appropriate, engaging relevant stakeholders where these have Trust wide impact.
