Job overview

Meadow House Hospice provides specialist palliative care for adults across Ealing and Hounslow through our inpatient unit, community teams and developing Day Service. We are seeking an experienced Staff Nurse to join our Day Services team, supporting patients attending for planned assessment, intervention, education and symptom management.You will work closely with the Clinical Nurse Specialist and wider multidisciplinary team to deliver high-quality, compassionate care for patients with complex palliative needs, often at an advanced stage of progressive illness. This role offers the opportunity to contribute to an evolving Day Services model, supporting timely interventions, advance care planning and collaborative working across hospice and community settings.

The successful candidate will have:

  • Experience in Specialist Palliative Care and/or Oncology, including end of life care
  • A compassionate, empathetic approach to patient and family care
  • Ability to work effectively with the MDT and volunteers
  • Ability to remain calm and professional under pressure
  • Ability to deputise for the CNS when required
  • Self-motivation and ability to work independently
  • Commitment to ongoing professional development
  • Strong organisational and IT skills

Main duties of the job

  • Delivery of nursing care to all patients in the Day Services setting
  • Supervises and coordinates patients, families and volunteers, providing direction and support in absence of the Day Services Manager/CNS.
  • Effective discharge planning.
  • Assist in meeting the requirements of the Clinical Governance programme pertaining to Day Services.
  • Ensures the assessment, planning, implementation and evaluation of patient care in the Day Services setting.
  • Acts as a resource and educator to staff, patients and their carers and pre-registered students in Day Services.
  • Ensures effective communication within the Day Services both among staff and with patients and their relatives and carers.
  • Deputises for the Day Services Manager/CNS as required.
  • Represents the Day Services at meetings internally and externally in absence of the Day Services Manager/CNS
  • Develops specialist skills and knowledge that are required to deliver palliative care in a Day Services setting
