Senior Staff Nurse  - Band 6

We are seeking to appoint a dynamic and enthusiastic candidate to the role of Senior Staff Nurse for our award winning Paediatric Hospital@Home service, based at the North Middlesex University Hospital site, to provide a high level service to all patients and staff within the department.

The service is commissioned to provide home based care to children in the borough’s of Haringey and Enfield. We are a 7 day service, 365 days a year, working hours 07:30-20:00, consisting of early and late shifts.

The H@H service is commissioned by NCL ICB and has provided positive patient experience and outcomes to children in their own homes, who otherwise would have been admitted to Inpatient units. The function of the team will be to provide hospital standard care to children with acute nursing needs in the community.

Working within a virtual ward model, the service will develop bespoke care pathways for children requiring nursing care which will avoid extended lengths of stay and admission. The team will work closely with Paediatricians and pharmacy colleagues to ensure a seamless transfer home and provide high quality nursing care according to their treatment plan.

Main duties of the job

The service currently covers the Borough of Enfield and Haringey, with possible plans for expansion. The role is ideal for an experienced paediatric nurse, with robust clinical skills who is able to work autonomously and as part of a team. Experience working in PED or PAU is desirable.

The BCYP H@H operates 7 days per week, 08:00 to 20:00. The service currently offers 7 care pathways, and it continues to develop new pathways. The Hospital at Home service sees babies, children and young people at home for treatment and assessments.

The post holder will work within the BCYP H@H Service, delivering treatment, reviews and assessments at the service user’s home. They will work within the multidisciplinary team to provide safe and appropriate management of children and young people with an acute illness which can be treated in the community. They will be working closely with the acute teams to support flow across the departments, facilitate early discharges and ensure care and treatment plans are met in the community.

The post holder will have vast experience in paediatric nursing to be able to identify patients suitable for H@H, complete triages, and to support patient flow and rapid discharges. The post holder should be able to plan, implement and evaluate patient care, as well as to follow and implement policies and guidelines.
