The work will focus on both admissions avoidance and supporting discharge home from hospital. In your new role you will work closely with Community Nurses, Assistant practitioners, support staff, and social workers to provide the best possible care to service users in their own homes.
We are a 7 day service - 8am to 6pm on rota basis. Self rostering is encouraged.
An exciting opportunity has arisen to join our developing integrated community services providing clinical triage, rehabilitation, falls management, admissions avoidance and support timely discharges from our acute Trust partners.
You will work autonomously as well as part of the multi-disciplinary team, liaising closely with health and social care colleagues to coordinate services, promote recovery from illness and enable health, wellbeing and self-care. You will be visiting patients within their own homes using a goal-orientated focus, to maximise patient’s independence, safety and quality of life.
The post will involve cross site working and community visits.
A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.
You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.
