Job overview

A new and exciting position has become available to work across the Hospital to Home Therapy Team and the Department for Older People's Medicine Inpatient Therapy Team at Aintree Hospital as part of a static internal rotation.

Timing of the rotation across both specialities will coincide with the external rotations across LUHFT.

Hospital to Home Therapy Team  (H2H)

The H2H team is made up of a mixture of Occupational Therapists, Physiotherapists, Assistant Practitioners and Therapy Assistants. The team accept referrals from the therapy inpatient teams across Aintree to carry out Discharge to Assess, Same Day/Next day follow up visits and Environmental visits in patient's own homes.

Older Peoples' Inpatient Therapy Team (DMOPS)

The DMOPS team is made up of Occupational Therapists, Physiotherapists, Assistant Practitioners and Therapy Assistants. The team covers 3 inpatient DMOPS wards as well as supporting an outpatient falls clinic. You will work closely with the AHP team and the wider MDT covering a DMOPS caseload, supporting patient discharges by carrying out vital OT assessments and interventions.  There may also be opportunity to support falls clinic by completing post falls assessments in  patients own homes.

This post is suitable for an  Occupational Therapist that has experience working both in acute inpatient settings and in the community.

Main duties of the job

Providing key OT assessment and treatment plans for patients in an acute  inpatient and community setting.

To carry out discharge to assess, complex follow up visits and environmental visits.

To manage designated case load with complex needs using evidence based/client centred principles to assess, plan implement and evaluate interventions.

Maintain records which fulfil legal requirements

To regularly supervise B5 OT's, B4 or B3 staff and OT students

To provide leadership for junior staff , through supervision and performance reviews.

To participate in the planning, development and evaluation of both services.

To contribute to the maintenance and development of the Trust OT service.
