Job summary

Main duties of the job

Currently referrals are received from assessment services for consideration. The team meet weekly to discuss referrals and either allocate accordingly or signpost or refer to more appropriate services.

Practitioners will manage their own caseload, devise a plan of care in partnership with the patient working with them for up to 10 weeks following the nursing process to ensure a high standard is achieved.

We are a specialist NHS Foundation Trust that provides community, mental health and learning disability services for the people of Barnsley, Calderdale, Kirklees and Wakefield. We also provide low and medium secure services and are the lead for the west Yorkshire secure provider collaborative.
