Main duties of the job

To work as part of an integrated health and social care team providing a crisis intervention and early supported discharge service for patients across the City of Southampton registered with a Southampton GP practice.

To have day-to-day responsibility for a designated caseload of clients requiring nursing/ rehabilitation/care management intervention and to organise and prioritise the required programmes of treatment (cases will be reviewed periodically by a more senior member of staff)

Work within the community and in University Hospital Southampton ED as a lone practitioner managing situations of risk with telephone support. Seek support and advice with cases from more experienced colleagues when faced with matters outside sphere of responsibility, knowledge, or skills.

Obtain informed consent from client to treatment and work within agreed guidelines for clients unable to give informed consent. Obtain consent to the sharing of information and document all consent issues in accordance with relevant Trust and Local Authority procedures.
