An exciting opportunity has arisen for Senior Community Mental Health Practitioner to join our Assertive Outreach Team in York.
This is an exciting opportunity for experienced practitioners to work as part of a multi-disciplinary team, specialising in work with adults who experience severe and enduring mental health, specifically psychosis and dual diagnosis.
Applications are welcome from those with Nursing, Social Work or Occupational Therapy registrations.
Nursing candidates must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework at the time of applying.

As a registered professional this is an ideal opportunity for someone who wants to develop their leadership and clinical skills in a community setting working within assertive outreach. This post is a secondment or fixed term opportunity; the contract will terminate August 2027.
As a core member of the team, you will also be responsible for your own caseload, providing assessments, care co-ordination and delivering intervention in the community for individuals who have severe and enduring mental health who are difficult to engage. Service users under our caseload are often marginalised from society and experience a range of socio-economic deprivations and have experienced trauma.
You will demonstrate excellent assessment skills, interpersonal, and team working skills and be able to work collaboratively with a range of statutory and third sector agencies. The team operate Monday-Saturday 9-5 therefore you will be required to work occasional Saturday shifts.
You will have an enthusiastic approach towards your own personal and professional development. In return, you will receive regular appraisal, clinical, management and caseload supervision and will benefit from the support of an experienced and committed multi-disciplinary leadership team.
