Community Mental health Team in Central Norfolk

Come and join a great team who care about service users and each other.

Working with people with moderate to severe mental illness who require specialist secondary care services as well as people with dementia and comorbid mental illness..  The team provides a  comprehensive service and works with a variety of other services to connect people to the support they need.

You will be based at Gateway House in Wymondham and work as part of a team  assessing and treating people people with moderate to severe mental illness who require specialist services and whose needs cannot be met elsewhere in the local system or by other partners.. The team provides diagnoses and formulates holistic, person-centred care and treatment.  You may be required to undertake assessments in a patients own home based on clinical need and you need to be wiling to travel across South Norfolk.

Main duties of the job

The post holder will assess people with moderate to severe mental illness There will be some memory assessment work in the role.  The postholder will develop treatment plans with support from an experienced team that includes Consultant Psychiatrists, Clinical Psychologists, Senior Nurses and an Occupational Therapist.   The postholder will ensure service users have a clear recovery and discharge plan from the beginning of treatment.

  • The post holder will undertake the planning and delivery of direct patient care with appropriate supervision, ensuring continuity of practice and working within a multi-disciplinary and multiagency environment.
  • Support and advising to other health care professionals. To support aspects of service development.
  • Provide an appropriate learning environment for relevant students.
