The newly created Digital Health Service will be responsible for the clinical assessment and monitoring of Virtual Ward patients alongside the senior clinical assessment and care navigation of the acutely unwell adult that has been referred via the Urgent Single Point of Access.
The Senior Clinical Assessor/Navigator role will have dual function:
The role will require clinical skills of assessment, alongside the ability to utilise digital equipment and telephony systems to support the role within the Digital Health Service.
Collaborate with referring HCP and provide the appropriate delivery of patient care and clinical and professional advice that will result in effective navigation of the patient to the appropriate service
Develop clinical skills in the assessment of acutely ill patients via the Digital, remote technology.
Provide the delivery of evidence based nursing care by providing expert clinical assessment, care planning, care delivery, care evaluation and clinical advice in accordance with NMC Code of Professional Conduct.
The post holder will work as part of the Digital Hub team and after accepting the referral will ensure that the correct triage tool is utilised to support effective care navigation to identified services within Stockport. The service Standard Operating Procedure will be adhered to alongside use of the Directory of Service DOS
The post holder will ensure the patient is navigated to the correct department and team ensuring electronic records are contemporaneously completed and referral is accurately transferred.
The post holder will be responsible for direct bookable appointments into identified services via Adastra and be compliant with the protocol and guideline for its use.
If appropriate will signpost the referrer or patient to appropriate alternative services.

If successful this vacancy will require Enhanced DBS check at a cost of £54.50. This cost will be deducted from your first Trust salary
In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
