Job overview

The Senior Children’s Community Nurse (SCCN), will support and contribute to the development of the Integrated Community Paediatric Service (ICPS) , helping to ensure that policy and practice is developed and delivered in line with Trust, regional and national best practice.

The post holder will work as an autonomous nursing practitioner whilst being part of the bigger team offering to support for CYP and families whilst having an acute episode of illness. The SCCN will work collaboratively with team members, other professionals and agencies in the assessment, implementation, and evaluation of community nursing care.

Provide and promote evidenced based knowledge and skills to manage the health outcomes for CYP and their families and carers across a variety of community settings to safely meet the CYP health care needs.

Main duties of the job

  • 1    To deliver evidence based nursing care to children and young people (CYP) in a variety of community settings by providing clinical assessment, planning, delivery, evaluation of care and clinical advice in liaison with the team leader, senior staff, peers and or multi-disciplinary professionals. 2    To act as patient’s advocate in promoting, and delivering the highest standard of nursing care, ensuring all fundamentals of care are addressed and current, relevant policies and procedures are adhered too. 3    To recognise and monitor acute exacerbations of illness and initiate appropriate nursing support and action any urgent medical interventions. 4    Work collaboratively with the hospital inpatient areas, Childrens ED, Observation and Assessment unit and primary care to facilitate early and prompt discharge and deflect unnecessary admissions to secondary care.
