JOB TITLE Specialist Diabetes Dietitian

Lead Dietitian
Assist the Lead Dietitian in the development and management of a quality Nutrition and Dietetics Service throughout the Trust and particularly within the specialist area diabetes.
Liaise with relevant staff to promote good nutritional care for all patients.
Act as a source of expertise for professional colleagues in these specialist areas of work and ensure the dissemination of up to date information on diet therapy to all relevant staff.
Assess and treat patients who may have complex and/or chronic presentations and determine independent nutritional diagnosis/diagnoses and resultant diet therapy indicated.
Work independently as a Specialist Diabetes Dietitian.
Operational Delivery;
Calculate individual dietary regimens in accordance with dietetic assessment for inpatients and outpatients, care home and domiciliary patients
Co-ordinate staff to ensure that patients on diet therapy receive suitable menus, and that the meals served are in accordance with their diet therapy needs
Counsel patients and/or their relatives as appropriate and review their progress
Identify, develop and evaluate dietary information leaflets in agreement with Lead Dietitian.
Ensure the availability of stocks of prescribable foods, identified as "Borderline Substances".
Ensure accurate and consistent dietary advice is provided to patients and carers by members of the Health Care Team through up to date evidence based nutritional education.
Deliver group education sessions to patients and carers
Provide appropriate and tailored advice to patients with life limiting illnesses who require complex symptom management as an integral part of the Multidisciplinary Team.
Maintain professional standards and be willing to undertake professional updating.
Contribute to the development of and comply with Nutrition and Dietetics and Trust strategies, policies, procedures, guidelines, protocols and codes of conduct.
Participate in the Trust’s KSF Appraisal Conversations process.
Provide cover for other dietitians as required throughout the Trust.
Ensure effective and economic use of resources.
Contribute to the development of educational displays and participate in health improvement education initiatives.
Participate in the education of groups of patients and health professionals as deemed appropriate by the Clinical Lead Dietitian and Lead Dietitian.
Participate in recruitment and selection panels as appropriate.
Ability to work unsocial hours including availability for seven day working as required

Information Management;
Keep legible, comprehensive and accurate written records to agreed Standards and ensure patient confidentiality is maintained.
Ensure statistical and other information is collected, updated on databases and made available for service evaluation.
Key Working Relationships;
Liaise with Medical, Nursing and Catering Staff, Social Workers, Pharmacists and other allied health professionals as appropriate.
Work as a member of the relevant Multidisciplinary Teams across the Trust and promote good interpersonal relationships within the team to achieve good nutritional care for all patients.
Provide training and supervision of junior dietetic staff and students as appropriate.
Participate in dietetic team, dietetic service and multi-disciplinary team meetings
Represent the service at specialist meetings as required and in agreement with the Clinical Lead Dietitian and Lead Dietitian
Ensure effective communication.
Quality
Lead and participate in clinical practice development and quality improvement projects.
Monitor the quality of service by Clinical Audit and investigate in partnership with the core dietetic team and/or within their specialist role or multidisciplinary team
Maintain professional registration with the Health and Care Professions Council
