Healthcare in Prison aims to provide innovative, compassionate and patient centred care to improve the outcomes of those in Prison, with a population health focus. Working in partnership with the extensive multidisciplinary team, the post holder will use clinical pharmacy skills to improve safety, efficiency, effectiveness and consistency of prescribing within Healthcare in Prison. This may include working within the areas of primary care, mental health and addictions.
To assist in the provision of a comprehensive, high quality and efficient pharmacy service to the Prison Healthcare. The post holder will work as an effective member of the pharmacy team providing comprehensive pharmaceutical services, including Medicines Management and dispensary duties.
Improving Safety
1. Clinical/Professional/Specific Managerial Responsibilities
· Ensure the appropriate interpretation of prescriptions prior to their preparation in the pharmacy.
· Provide accurate and appropriate medicines information in response to internal enquiries, including patient and healthcare worker enquiries, utilising all available resources.
· Accurately prepare extemporaneous preparations utilising appropriate equipment whilst ensuring adherence to all relevant good manufacturing guidelines and COSHH.
· Provide a prescription monitoring service including identification of adverse drug reactions, drug related events, drug interactions and rationalisation of medicines.
· Provide an accurate final check on dispensed medication within the pharmacy.
· Obtain a comprehensive and accurate admission medication history at committal stage to ensure that patients receive the correct and appropriate medicine in a timely manner.
· Assess and manage patient’s own drugs to ensure effective prescribing and patient safety at all times.
· Assist in the development of pharmaceutical care plans in accordance with patients’ diagnosis and assess treatment options available.
· Provide medication counselling and education to patients. This includes the provision of written or verbal information to patients with difficult behaviour or those with impaired cognitive function or impaired senses.
· Provide medicine related information and advice of a therapeutic, legal or procedural nature to prescribers and other healthcare staff ensuring effective prescribing and patient safety at all times.
· Assist in the collection and analysis of workload data to identify trends in prescribing patterns and encourage cost efficiency.
· Assist in ensuring working practices and the working environment conform to statutory requirements, trust and local policies with regard to Health and Safety issues.
· Assist in the development and introduction of new technologies and working practices designed to improve the efficiency and running of the section
· Contribute to effective and economic use of resources and the maintenance of all equipment.
· Undertake risk assessments and propose ways of managing/reducing risk within pharmacy.
· Assist in the production of expenditure reports when required.
· Assist in maintaining, updating and developing in-house documentation and standard operating procedures in accordance with published standards and local policies.
· Participate in audit/research projects as required.
· Participate in the implementation of policies and procedures for the safe and efficient use of medicines e.g. Pharmaceutical and Clinical Effectiveness Programme.
Although currently service provision is Monday to Friday, the candidate may be required to work across a weekend, based upon service need .

Improving Efficiency and Cost Effectiveness
Assist practices in delivering Department of Health and SPPG priorities and plans for medicines optimisation, health and well-being.
Reduce levels of prescribing expenditure as appropriate in accordance with targets.
Utilise relevant software for the recording of the interventions carried out and contribute to the development of the systems for intervention recording
Governance
Undertake regular clinical audit.
Participate in organisational audit as required

Be aware of the legal and professional issues pertinent to working as a practice based pharmacist including duty to maintain effective registration and comply with requirements for fitness to practice; accountability and delegation.
