JOB SUMMARY

In order to meet the overall mission of the Trust the post holder will

Work as a Clinical Pharmacist providing a clinical pharmacy service to inpatient wards and clinics

Band 6/7 Pharmacists will either

Gain rotational experience across unscheduled care (including emergency department), medical, surgical and other clinical specialties.

Specialities include ICU, medical day case, biologic therapies, mental health, gastroenterology, cardiology, palliative care, respiratory, maternity, paediatrics, orthopaedics, general surgery, hospital at home (based in Ards hospital) and medicines & patient safety.

Rotations will depend on service needs, typically 12 months in duration.

Or

Work in the Emergency Department as a band 6/7 pharmacist.

Ensure the safe, effective and economic use of medicines

KEY DUTIES / RESPONSIBILITIES

Main Responsibilities

Complete the Foundation Programme or equivalent (FP2 or original format FP) within 12 months of appointment to allow progression to a band 7.

Required to undertake and attain the prescribing qualification within first 2 years of appointment.

To practise as a generalist Independent Pharmacist Prescriber during the patients stay and at discharge.

To either

Participate in the band 7 clinical pharmacy rotational experience across unscheduled care (including emergency department), medical, surgical and other clinical specialties. Specialities include ICU, medical day case, biologic therapies, mental health, gastroenterology, cardiology, palliative care, respiratory, maternity, paediatrics, orthopaedics, general surgery, hospital at home and medicines & patient safety.

OR

Work in the emergency department as a band 6/7 clinical pharmacist.

Provide a high level of clinical pharmacy input, as part of the multi-disciplinary team.

Provide high level of medicine and patient safety information to relevant stakeholders, as part of the multi-disciplinary team.

Develop good working relationships with the other members of the healthcare team, negotiating and influencing changes in prescribing practice to promote evidence based practice and improve medication and patient safety.

Obtain a comprehensive and accurate admission medication history to ensure that patients receive the correct and appropriate medicine in a timely manner.

Optimise patients’ medication during admission.

Manage patient’s own drugs to ensure effective prescribing and patient safety at all times.

Implement pharmaceutical care plans in accordance with patients’ diagnosis and assess treatment options available.

Provide a prescription monitoring service including identification of adverse drug reactions, drug related events, drug interactions and rationalisation of medicines during hospital stay.

Work in conjunction with the medical staff to prepare timely discharge prescriptions

Ensure timely discharge information is available to primary care healthcare professionals.

Provide medicine related information and advice of a therapeutic, legal or procedural nature to prescribers and other healthcare staff ensuring effective prescribing and patient safety at all times.

Take a role in the development of an integrated clinical pharmacy service and medicines optimisation in light of national, regional and local standards.

Help develop, implement and monitor regional and local policies, protocols, guidelines and initiatives in the area of medicines management.

Undertake risk assessments to ensure any potential medication incidents are minimised.

Assist in benchmarking the clinical pharmacy service and contribute to the development of performance indicators to monitor effectiveness and patient benefit

Support the medication and patient safety team in all work relating to medication and patient safety as required.

Provide advice and counselling to patients and their carers on the proper and informed use of medicines prior to discharge. This includes the provision of written or verbal information to patients with difficult behaviour or those with impaired cognitive function or impaired senses.

Promote optimal, safe and appropriate drug use and high quality prescribing practice through development of local formularies, drug usage review, development of prescribing guidelines and multidisciplinary staff training.

To participate in Medical, Nursing and allied professional education as appropriate and in the rehabilitation education of patient groups.

Carry out intervention monitoring according to local guidelines to ensure the safe, effective and economic use of medicines.

Attend multidisciplinary meetings as appropriate and provide timely analysed drug usage reports with recommendations on budgetary and formulary activities.

Proactively contribute to therapeutic decisions concerning patient management.

Advise the Clinical Leads and Head of Pharmacy and Medicines Management of budgetary changes due to possible alterations in the use of drugs e.g. new licensed indications, new literature and new drugs marketed.

Initiate and participate in pharmaceutical aspects of clinical audit, research and supervise students where appropriate.

To participate in the planning and administration of drug trials.

Participate in relevant Continuing Professional Development (CPD) and apply knowledge gained to current and future work as appropriate.

Collate and monitor information on the expenditure of high cost drugs to ensure cost effective prescribing.

Participate in the mentoring, teaching and training of band 6 pharmacists on the Foundation Programme, foundation training year pharmacists and university students.

Participate in the mentoring and teaching of pharmacy technicians on the Medicines Management Accredited Programme (MMAP)

Supervise and support the junior pharmacists and technical staff working within your area on a daily basis

Ensure the appropriate interpretation of prescriptions prior to their preparation.

Provide accurate and appropriate medicines information in response to internal and external Trust enquiries including patient/carer/healthcare worker enquiries utilising all available resources.

Plan and organise own work according to agreed objectives and timeframes for the designated area/pharmacy service.

Accurately prepare extemporaneous preparations utilising appropriate equipment e.g. fume cabinet, balances etc. whilst ensuring adherence to all relevant good manufacturing guidelines and COSHH.

Assist in the review and evaluation of pharmaceutical clinical technologies (drugs, dressings).

Collect and collate all workload stats.

Report incidents and near misses in line with Trust policy

Encourage staff to report near misses and incidents, share the learning and undertake reflective practice

Encourage pharmacists, medical and nursing staff to report adverse drug reactions

Participate in the preparation of conference presentations, posters, writing reports and the writing up of publications.

Participate in the implementation of policies and procedures for the safe and efficient use of medicines/Pharmaceutical Clinical Effectiveness Programme.

Participate in the recruitment, selection and appointment of staff as required

Ensure the safe and secure storage of medicines in accordance with all medicines legislation and Trust security policies.

Participate in the opening/lock up, weekend and statutory duty rota and when required, on the on call rota.

Clinical/Professional/Specific Managerial Responsibilities

Support the Trust in its drive for seven day working in order to ensure it best meets the needs of our patients.

Hold a current full driving licence, which is valid for use in the UK, and have access to a car on appointment.

Able to attend the Ulster Pharmacy within 45 minutes if called to attend whilst on call.
