Band 4 SACT (Systemic Anti-Cancer Therapy) Pathway Coordinator

PLEASE NOTE: This role will require you to be able to work across all three SACT units in Reading, Bracknell and Thatcham.

The post holder will be responsible for coordinating the patient Systemic Anti-Cancer Therapy (SACT) pathway within the outpatient setting.

Working as part of a multidisciplinary team, you will support timely and safe planning, scheduling, and delivery of care for patients undergoing SACT treatments.

This role ensures patients have a seamless journey from referral through to treatment, promoting high standards of patient care, data integrity, and adherence to cancer waiting time targets and local performance metrics.

Main duties of the job

Patient Pathway Coordination

  • Act as the central point of contact for patients on the SACT pathway, ensuring a coordinated and efficient journey from consultation to treatment.
  • Schedule and maintain SACT appointments in line with treatment protocols, booking availability, and patient needs.
  • Work with pharmacy and nursing teams to ensure timely preparation and delivery of SACT.
  • Coordinate referrals, pre-assessment appointments, pre-treatment tests (i.e. GFR bloods, DYPD testing, ECHOs etc.), and ensure readiness for treatment.
  • Escalate delays or capacity issues, including potential breaches to the SACT CNS and/ or SACT Unit Managers as required.
  • Any other duties deemed appropriate to the role to support the increasing service needs as requested by the SACT CNS, SACT Outpatient Unit Managers or Lead SACT Nurse.
