Respiratory Clinical Nurse Specialist / Practitioner (Band 6)

An exciting opportunity has arisen to join our innovative Respiratory Services team as we expand to deliver an Acute Respiratory Virtual Ward. With newly secured funding, we are seeking a motivated and experienced Band 6 Clinical Nurse Specialist / Practitioner to help shape and deliver high-quality respiratory care across East Berkshire.
You will work within a supportive multidisciplinary team, providing patient-centred care across community and hospital settings. As a skilled Respiratory CNS, you will use advanced clinical, communication and interpersonal skills to assess and manage patients with respiratory conditions, particularly COPD and those requiring oxygen therapy. Care delivery will reflect patients’ social, cultural and spiritual needs.
Key Responsibilities: • Coordinate and deliver high-quality care for patients with respiratory illness. • Facilitate safe discharge of suitable COPD patients and support admission avoidance. • Assess, initiate and review home oxygen therapy across East Berkshire. • Provide specialist respiratory advice to Hospital Trusts, Berkshire East Community Health Services (BECHS) and GP practices. • Support service development towards an integrated, consistent respiratory pathway.

You will deliver specialist respiratory assessment and intervention for patients with a wide range of acute and chronic respiratory conditions. Working collaboratively with nurses, physiotherapists, nursing associates, associate practitioners, technicians and administrative colleagues, you will contribute to the development of holistic, evidence-based management plans tailored to individual patient needs.
The role involves assessing patients with complex physical and psychological presentations, planning and implementing personalised treatment programmes, and supporting the wider team in delivering safe, effective care.
