Job overview

We have an exciting opportunity to join our Newton Abbot Community Hospital therapy team as a part time band 3 Rehabilitation Support Worker. This is as a fixed term position for a period of 12 months to cover maternity leave.

This post offers the opportunity to support the delivery of rehabilitation to patients under the direction of qualified Physiotherapists and Occupational therapists on our inpatient medical wards in a community hospital setting. Our patients require ongoing therapeutic intervention to support their discharge home or to an alternate location. Our patients are predominately older, frail patients who require a supportive and encouraging approach to allow them to achieve what is important to them.

You will work alongside other members of the multi-disciplinary team including nurses and GP’s. We currently provide a therapy service on a Saturday, and you will be required to work approximately one Saturday in five.

This post offers 26 hours per week.

Main duties of the job

Delivering rehabilitation programmes to patients with the aim of increasing their independence and facilitating discharge from hospital under the direction of qualified therapy staff. This will include Physical exercise programmes, functional activity practice and cognitive interventions.

Supporting patients with the provision of equipment to support their independence.

Supporting patients at the time of their discharge. This may include attending the patients property as part of home visits or on the day of discharge.
