Rehabilitation Support Worker Band 3, Merton Enhanced Home Based Rehabilitation Team , Central London Community Healthcare NHS Trust (CLCH) - Merton

We are seeking an enthusiastic Band 3 Rehabilitation Support Worker to join our Community Discharge to Assess service working alongside Physiotherapists and Occupational Therapists.

We are looking for team players with a positive attitude and commitment to providing excellent care and interventions. Our goal is to optimise our patients’ independence and wellbeing, enabling them to return to their own homes following a rapid decline through illness or injury, including falls.

PLEASE NOTE THE FOLLOWING:

THIS ROLE IS NOT ELIGIBLE FOR SPONSORSHIP - due to being fixed term contract that ends 31st March 2026

SHIFT PATTERN - either 8am to 4pm  OR  2pm to 10pm over a 7 day period

APPLICANTS MUST HOLD a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)

Main duties of the job

The Rehabilitation Support Worker (RSW) functions as part of the Multi-Disciplinary Team to provide a seamless service to a group of patients under the guidance and supervision of an Allied Health Professional.

  • The RSW will be expected to become proficient and competent in their area of practice by undertaking competency assessments specific to the area of clinical practice.
  • The RSW will promote optimum independence and individualised patient-centred care to a delegated caseload.
  • The RSW will also support the team to deliver against key performance indicators and maintain quality of the service
  • The RSW will assist in identification of the health and social care needs, and participate in the delivery and evaluation of care packages developed to meet those needs under the direct supervision of a registered health professional.
  • The RSW will  work as part of an interdisciplinary team within community rehabilitation services to assist in the development of good working relationships with statutory, voluntary and independent organisations.
  • The RSW will able to provide competent and at the same time compassionate care to all patients with physical and cognitive difficulties within the community and /or care home settings.
  • The RSW will  support Senior Staff by able to collaborate with colleagues across own and other community teams ,acute teams, GPs, social services and voluntary sector to optimise patients journey, care, wellbeing and outcomes
