Job summary

We have an exciting opportunity for a Band 3 Rehabilitation Assistant to join our service, delivering high-quality, person-centred care to patients in their own homes and community settings.

You will work as part of a multidisciplinary team to support the delivery of rehabilitation programmes, helping individuals to achieve their goals and maintain independence. You will also contribute to supporting discharge from hospital by delivering rehabilitation interventions in patients' own homes where appropriate.

You will work with a range of patients under the guidance of a qualified therapist, including those with more complex needs. You will follow prescribed treatment plans and support with monitoring progress.

You will be supported by experienced community therapists and will follow guidance to deliver rehabilitation programmes within individuals' own homes or healthcare settings.

You will receive regular supervision, training, and the opportunity to develop your skills within a supportive team environment.

This post will be based within a community locality across the New Forest or Eastleigh areas.

Main duties of the job

Work under indirect supervision from the Therapy Team Leader, Senior Therapists, and Therapists being responsible for a delegated workload.

Participation in treatment activity

Dealing with distressing/sensitive issues

Ensure patients and carers are an integral part of their treatment planning and goal setting.

Be responsible for issuing of standard aids, whilst demonstrating an awareness of local cost implications.

Ensure that up to date written and electronic records, reports and activity data are maintained in accordance with professional and Trust standards.

Provide intervention in a variety of environments, e.g. client's home, hospital/healthcare settings, schools, work environment to meet client needs.

Take part in procedures to ensure smooth running of the team such as patient registration, liaison reporting and recording data etc.

Adhere to national and local manual handling guidelines.

Support the work of the registered practitioners in the team. To prepare patients for and assist with clinical/therapeutic activities or treatments.

To provide a Therapy Service and develop continuity of care under the supervision of a Supervising Therapist within the laid down policies and procedures/code of ethics.

To be responsible for the health and safety and welfare of patients/clients and others, including the care/use of equipment provided.

To lone work adhering to local and HIOW lone working guidelines.
