Main duties of the job

Franszika Willer ward is a 26 bedded rehabiltation ward. we also specialise in Palliative and End of life care. We accept patients from the acute sites, direct from accident and emergency, from other organisations and from the surrounding community

The main duties of the role are:

-Plan and deliver high quality of care to patients adhering to the trust policies and procedures.

-Maintain a high standard of documentation

-Demonstrate excellent communication skills both written and verbal

-Plan and deliver safe admissions and discharges

- Be responsible for the delegation of tasks to other members of the team

-Work well as a team player but also show initative working alone

Job responsibilities

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
