
The Support to Go Home Service is looking for a ‘Reablement Support Worker’ to join their team.
This is an exciting opportunity for someone with previous experience of working in a health or social care environment or you are looking to make a change to your career pathyway and would like to move to care and support patients in their own home in their safe recovery. As a reablement support worker you will be trained in a range of skills to deliver care and support in the community via the delivery of a personalised shared care and support plan. This will enable people to return home safely following a discharge from hospital without a delay, a timely return home after a visit to the emergency department or help to remain at home following a short illness with increased support.

Within this exciting role you will have the opportunity to support patients to become more independent with daily living tasks following their discharge from hospital, this includes supporting with personal care, meals, and mobility. You will also have the opportunity of supporting and providing palliative care for patients that are at the end of their life
As this role is supporting patients in their homes, you will be required to travel across West Suffolk. As frequent travel is essential for this role, therefore you must hold a full UK current driving licence. You will be asked to show a copy of your original driving licence at interview.
We can support full time and part time working, and as the service works over 7 days, we can offer flexibility with either early or late shifts between the hours of 7.30am – 10pm. You will be expected to be able to work one weekend in three.
We want to support your career in the NHS and can support you with training opportunities via a development program that can see you progress to a Band 3 Senior Home Care Support Worker upon achieving competencies within your role.

STAFF CAR PARKING
Staff Car Parking on the West Suffolk Hospital Site is eligible for individuals working outside of the core hours, 07:00 – 18:30.
For individuals working within the core hours, the Trust offers parking off site at the Bury St Edmunds Rugby Club, which is serviced by a regular shuttle bus to and from the Hospital. Details of the bus timetable can be found in the on-line starter pack on the West Suffolk Hospital website.
This parking restriction does not apply if an individual is required to work on a weekend or if they are required to complete “on call” duties within their role.
