Upon completion of the initial assessment the post holder will identify the mental health cluster that the client will be assigned to in accordance with the mental health clustering tool.
The post holder will deliver evidence-based interventions via differing modalities, such as; a group setting, face to face, video chat and telephone.
Operate at all times from an inclusive values base which promotes recovery and recognises and respects diversity.
Educate and involve family members and others in treatment as necessary, where the client has given permission.
Work closely with other members of the team ensuring appropriate step-up and step-down arrangements are in place to maintain a stepped care approach.
Manage and convey information which may be of a sensitive and/or distressing nature.
Analyse and interpret patient outcome measures such as PHQ9 and GAD7 and to then administer the appropriate evidence-based intervention, monitored through caseload management.
Assess clinical risk and refer to specialist mental health services as indicated autonomously if assessed as urgent, or in collaboration with the caseload manager if the risk is assessed as low but the client's needs are beyond that of brief psychological interventions.
Make decisions on suitability of new referrals, adhering to the department's referral protocols, refer unsuitable clients on to the relevant service or back to the referral agent as necessary or step-up the person's treatment to high intensity psychological therapy.
