As a Practice Facilitator you will be focusing on the HCSWs. You will be working as a member of the ASCR Ward Practice Facilitator Team.

You will be responsible for:

  • Facilitating education pathways for HCSWs band 2 and 3 of varying ages and abilities within the ASCR wards.
  • Facilitating education and guidance involving one to one support and supporting their assessors with the assessment of competencies in practice.
  • Working within in policies and procedures and providing internal quality assurance as required.

You will also work alongside others in a similar role in practice and the practice facilitator team to help develop a workforce with the skills, knowledge, and competence necessary to deliver effective patient care and to enhance the quality of the service.

Main duties of the job

  • Excellent interpersonal and effective communication skills. Communicate sensitive information with empathy to HCSWs, assessors and their managers.
  • Work alongside HCSWs in the clinical environment according to required standards and to escalate any concerns.
  • Work closely with ward managers to support HCSW performance, conduct and professionalism.
  • Assess clinical skills.
  • Ability to work to deadlines agreed by education leads, practice facilitators and prioritise own workload.
  • Manage time effectively and problem solve.
  • With guidance from the practice facilitator team, work autonomously and be accountable for a cohort of HCSWs.
  • Work well independently and as part of a team.
  • Motivated.

Detailed job description and main responsibilities

  • Indentify HCSWs who are failing to meet targets.
  • Report any concerns to the line manager identifying issues outside their level of competence.
  • Planning, preparing and teaching.
  • Support wider Practice Facilitator Team within scope of practice
  • Prioritise areas for development, identify current performance requirements relevant to own practice and stay up to date with current practice.
  • Seek feedback, collect information and continually reflect on own performance.
  • Record keeping.
  • Maintain mandatory training in line with Trust policy.
  • Follow Trust policies.
  • Undertake relevant teaching courses.
  • Act as an excellent role model.
  • Work with managers to support HCSWs performance, conduct and professionalism.
  • Demonstrate excellent customer care.
  • Act within limits of own competence.
