We are looking for a motivated Band 6 Physiotherapist to join our Intensive Home Support Service (IHSS), delivering rapid, high‑quality therapy to help people remain safe and independent at home. You will complete holistic functional and environmental assessments, provide musculoskeletal and respiratory input, prescribe essential equipment, and contribute to preventing avoidable hospital admissions.

Working as part of a supportive multi‑disciplinary team, you will manage a varied caseload, work autonomously in community settings, and play an active role in daily MDT discussions.   • Deliver evidence‑based physiotherapy interventions for patients with acute, chronic, and complex needs.
  • Undertake musculoskeletal and respiratory assessments and formulate treatment plans using advanced clinical reasoning.
  • Prescribe and fit low‑level and urgent moving and handling equipment to maximise independence.
  • Work autonomously in community settings, including lone working with MDT support.
  • Contribute to daily MDT discussions and work closely with Hospital at Home and wider health and social care partners.
  • Monitor, evaluate, and adapt treatment plans in collaboration with patients and the multi‑disciplinary team.
  • Maintain accurate clinical records and ensure safe use of equipment.
  • Support supervision, training, and development of junior staff and therapy assistants.

  • Prescribe, fit, and demonstrate low‑level and urgent moving and handling equipment to maximise independence and safety at home.
  • Work autonomously in community and domiciliary settings, including lone working with MDT support.
  • Contribute to daily MDT discussions and collaborate closely with the Hospital at Home service.
  • Play an active role in delivering the 2‑hour UCR response, supporting rapid intervention to avoid unnecessary hospital admissions.
  • Monitor, evaluate, and adapt treatment plans in partnership with patients, carers, and the wider MDT.
  • Maintain accurate clinical records and ensure safe use of equipment in line with professional and organisational standards.
  • Support the supervision, training, and development of junior staff, therapy assistants, and students.
  • Participate in service development, clinical governance, quality improvement, and audit activities.
  • Promote self‑management strategies and provide education to patients, carers, and other professionals.
