Main duties of the job
- Provision of a quality service, demonstrating reliability in completing medication drug histories and skill in non-routine tasks e.g. advising patients/carers on medication issues or handling routine drug queries within communities and GPs
- Development of and participation in Community Mental Health Team duties to include medicines reconciliation, assessment of PODs, attending multi-disciplinary meetings, and liaising with other healthcare professionals to gather relevant information on current medication and compliance status of patients
- Assistance in the review and management of the shared care procedures within the Wigan community teams.
- Contribute to development and implementation of policies and procedures within the department and those impacting across services
- Supervise and train new starters within Wigan communities in the provision of an effective and efficient pharmacy services.
