Job overview

We are looking for a registered Pharmacy Technician to join our team based in Tonbridge Cottage Hospital.

Our Pharmacy Service actively tries to ensure that medicines are optimised for individual patients, that they are empowered to manage their own medications whilst in Hospital and educated to prevent avoidable future admissions and keep them safe in their own normal place of residence. Discharges are paramount and collaborative working across healthcare sectors plays a key role in keeping patients safe.

Working with the wider Multidisciplinary Team (MDT) our patient centred approach is demonstrated by all those in the Pharmacy Team from the moment the patient enters the organisation, through their Community Hospital admission to their safe discharge.

Main duties of the job

You will provide a comprehensive medicine management service to the community hospital wards.

  • Making judgments on how to manage workload
  • Completing comprehensive medicines reconciliation on admission including checking and documenting specified blood results and highlighting to pharmacist where further review of a patient’s medication is required and interpreting and applying information retrieved from medical and nursing notes with regards to medicines reconciliation and prescribing issues
  • Checking patient’s own medication (PODs) to aid in problem solving and to adequately manage medicines orders.
  • Managing the supply of medicines during admission alongside the Pharmacy Support Workers to reduce the impact of missed doses of medicines on a patients care.
  • Ensuring compliance with prescribing guidelines and with local formularies
  • Being involved discharge facilitation, supporting self administration of medicines, providing patient counselling, making appropriate referrals to support services to help ensure patients (or their carers) go home safely able

Additional Working Pattern: 22.5 hours over 3 days
