We are looking for a Peer Support Worker to join our Primary Care Mental Health Network in Holderness, to support individuals accessing Primary Care Mental Health Services.

The role uses lived experience of mental health and recovery to build supportive peer relationships that promote hope, empowerment, and self-management. The post holder will work as part of a Multi-Disciplinary team to support individuals in identifying strengths, setting meaningful recovery goals, and accessing appropriate services and community resources.

The role focuses on enabling individuals to take an active role in their recovery, improving well-being and supporting social inclusion.

Main duties of the job

  • Provide recovery-focused peer support using lived experience to inspire hope and promote self-belief •    Support individuals to identify strengths, develop recovery goals and contribute to Recovery Action Plans •    Signpost and support access to community resources, services, and social opportunities •    Work alongside the Multi-Disciplinary Team to support safe, coordinated care •    Share coping strategies, self-management tools and recovery approaches •    Support engagement with appointments, meetings, and activities where appropriate •    Promote strengths-based practice and recovery language within the team •    Maintain accurate records and work within professional boundaries and Trust policies
