Patient Pathway Coordinator Team Lead – Band 5 Gynaecology

Are you an experienced and motivated individual with a passion for improving patient care and pathway management? We’re looking for a Patient Pathway Coordinator Team Lead to join our dynamic team within the Gynaecology department.

In this pivotal role, you will work closely with the Assistant Specialty Manager to enhance the efficiency and effectiveness of patient pathways. This includes proactively managing Patient Tracking Lists (PTLs), addressing Lost to Follow-Up (LTFU) cases, and ensuring timely capture of clinical outcomes.

You will lead and support a dedicated team of Patient Pathway Coordinators, providing day-to-day management, training, and development to ensure excellent service delivery and adherence to national, local and Trust targets.

Key responsibilities include:

  • Overseeing and improving the performance of patient pathway processes
  • Collaborating with clinical and operational colleagues to resolve pathway delays
  • Monitoring data to identify trends and implement improvements
  • Line management of the Patient Pathway Coordinator team

Main duties of the job

Manage the departmental workload and demonstrate efficient organisation and leadership skills over the effective day-to-day operations of the secretarial workforce.   Work autonomously co-ordinating the workload of the team, providing full organisational support to the consultant’s practice and their team.

Ensure procedures and working practices are in place so that the Specialty and Trust can deliver a service that meets the standards and targets that have been set.  Provide a considerate, patient focussed service in all dealings with patients and staff around the Trust.

To provide support to the nurses and outpatient’s teams with room bookings and clinic management.

Provide day to day leadership of the secretarial team including, allocation of work and duties, enabling the team to provide an efficient and successful service to the clinicians.
