As a Patient Pathway Coordinator, you will play a pivotal role in ensuring the seamless management of patient pathways within our esteemed Diabetes service. You will be joining a highly efficient team dedicated to maintaining the highest standards of patient care and service delivery.

Key Responsibilities:

  • Be the point of contact for patients accessing elective hospital services throughout their pathway of care.
  • Work under the direction of the Senior Patient Pathway Co-ordinator / Speciality Consultant Team and provide expertise in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.
  • Provide a range of administrative support services to the speciality Consultant Team.
  • Ensure high levels of patient care and clinician satisfaction by being an accessible, customer-focused, and knowledgeable point of contact.
  • Adhere to Standard Operating Procedures (SOP) to ensure a consistent approach across the Trust, describing how the functions of this role will be carried out along with the timescales that will be adhered to.
