An exciting opportunity has arisen for a Band 5 Physiotherapist to join our fantastic Paediatric Therapies team in CCICP,  to work in Hebden Green School, Winsford.

The school supports children aged 2- 19 years, who have a range of complex physical needs affecting their movement and/or activities of daily living, including complex neurological conditions. It is an innovative, collaborative and fun place to work, allowing staff to build close working relationships with the children and young people and their families.

The team work closely together to create an integrated approach encompassing Paediatric Occupational Therapists, Therapy Assistants, other members of the MDT such as Speech and Language Therapists and Nurses, and education staff. You will have your own caseload and would be expected to work autonomously as well as part of a team, and would receive regular supervision from our experienced therapists, and close support from our locality leaders.  Over the school holidays there will be potential to visit the children in their own homes, and also potentially assist with a community caseload. It is therefore essential that you are a car driver.

Main duties of the job

- Work as part of an MDT to ensure that children and young people are supported to attend the school environment

- Complete assessments for the children and young people on your caseload

- Develop care plans for the children and young people on your caseload, and work with all therapy colleagues to implement care plans both in schools and at home.

- Regular liaison with members of the MDT

- Regular liaison with parents/carers to ensure that they receive feedback on their child's progress and assist in goals setting

- Active involvement in SEND process and completion of EHCP Annual reports

- Active involvement in Safeguarding including attending meetings, liaising with safeguarding teams, completion of referrals as appropriate, and liaison with social care as required.

- Assessment and implementation of equipment as required for children and young people.

- Participation in Physio peer review meetings, training for other professionals and education colleagues/parents or carers
