The Community Engagement Team (CET) is formed by Step & Thrive and VCS -Voluntary Care Sector organisations such as mental health and Employment services. CET is one of the pathways within the newly formed Integrated Front Door in Enfield which is the patient’s first point of contact when they are referred to mental health services. Also sitting within this Integrated Front Door is the Single Point of Access Team, the AARS Team and our Local Authority Partners.
The post holder will support in the delivery of community mental health interventions and/or activities provide by a RMN Mental Health Nurse, Community Psychologist, Occupational Therapist, gradual mental health workers and VCS partners. The delivery will be through either 1:1 or multidisciplinary group interventions, which are co-produced by the trust and system partners; supervision will be provided within appropriate the professional framework.
Additionally, under directions of the Community Service Manager, the post holder is expected to support community out-reach supporting service users to engage with services and within the wider community.
To communicate appropriate information to and from other department/ service areas as required.
To attend and participate in team meetings and contribute ideas to the multi-professional team.
To undertake a range of duties which involve direct patient care, under supervision following competency assessment.
To have informed knowledge and understanding in the psychological, social, physical and spiritual needs of all service users.
To use Dialog+ and other tools to assess service users health and social care needs
To contribute to the design of care plans in line with risk assessment and psychosocial needs.
To utilise designated recovery tool with outcomes focusing on care planning, ensuring clients understand they will work in a collaborative manner to engage in social inclusion activities such as seeking employment, education and independent living.
To support the assessment and management of patient care needs through initial data collection, monitoring of patients’ progress, feedback, discussion, and reports and in care reviews with the clinical team.
To provide information of health promotion and healthy living.
To provide patients and their carers with a service that is culturally sensitive and gives due consideration to any special needs.
To contribute to the effective communication process of service users, carers, family, friends and staff colleagues.
This is an exciting opportunity to work within a co-produced, innovative community mental health and well-being team. The Community Engagement team is for working age adults (18 to 65 years old). This service will provide accessible, community-based support interventions and activities (group and individuals) which, address the holistic well-being needs of service users (e.g., emotional, social, occupational, practical, etc). This is a co-produced service that blends psychological, occupational therapy, and evidenced based interventions supported by voluntary sector partners on out-reach, employment alongside lived experience to ensure that support is accessible, relevant, and effective. The service seeks to create community integration and support by working alongside stakeholders across the London Borough of Enfield.
