Job summary

Main duties of the job

  • Will undertake specialist respiratory assessment of patients nursing needs, plan, implement and evaluate care delivery using clinical knowledge and clinical skills.
  • Identify suitable patients for early supported discharge, plan and arrange discharge and provide individualised, holistic care plans to suit the patients needs.
  • Review patients in the home environment following referral for admission avoidance. You will undertake advanced clinical assessment obtaining a history, clinical examination and conclude diagnosis implementing a treatment plan. You will also make the judgement if the patient can safely be managed at home or if they require hospital admission.
  • Review patients in accident and emergency or ambulatory care unit for admission avoidance following an initial assessment by the medical team. If deemed suitable for home care you will plan and implement the patients discharge and arrange follow up at home.
  • You will undertake frequent home visits to patients who are receiving an acute episode of PERT care ensuring their clinical condition is stable, maintaining a safe alternative to in -patient hospital care.
  • You will ensure all documentation and communication is maintained to a high standard.
  • You will liaise with members of the multi-disciplinary team and refer to other services and health care professionals where appropriate ensuring holistic care.
