Job summary

Main duties of the job

Join us in making a real difference to the health and wellbeing of people across Darlington. In this role, youll recruit and support clients on their journey to becoming smoke-free, creating personalised stop smoking plans that include tailored medication or vape support, in line with local authority targets.

Youll work directly with individuals to treat tobacco dependency and promote harm reduction, carrying out client assessments, delivering evidence-based advice, and running regular support sessions. This will involve administering stop smoking treatments, following trusted clinical guidelines, and engaging with clients face-to-face across a range of community settings in Darlington.

Together with your clients, youll develop individualised support plans based on each persons unique needshelping them stay motivated and confident throughout their quit journey. Youll manage your own caseload of clients, guiding them every step of the way and tracking their progress.

Beyond one-to-one support, youll play an active role in community engagement, working within GP practices, local hubs, and other community spaces to raise awareness and connect people with the help they need.

Job responsibilities

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
