Our High Peak Older Adult Community Mental Health Team (CMHT) and Living Well Team in Buxton are excited to welcome a Band 6 Older Adult Specialist to our supportive and welcoming team.

We're looking for someone with experience working with older adults in the community who are living with functional mental health conditions. This brand-new role, based within the Older Adults CMHT, offers an exciting opportunity to shape meaningful support for individuals in our local area.

Main duties of the job

You'll be playing a key part in introducing people into the Living Well Team, helping to identify their needs and designing person-centred, compassionate support that empowers them to live their best life.

Collaboration is at the heart of this role. You'll be working alongside colleagues across the community to ensure that the right support is delivered at the right time by the right person.

About the Teams

While the role is supported by the Older Adults CMHT, you’ll also be working closely with the Living Well Team, a multi-agency group that places individuals right at the centre of their care. One of the most rewarding parts of this role is being part of a team that reaches out to people who may not have had access to mental health support before.

You will be

  • Clinically responsible for supporting the delivery of a flexible, responsive local service, taking forward an Integrated Wellbeing Pathway in the designated locality, which effectively supports the delivery of the Community Mental Health Framework.
  • Working collaboratively with colleagues, key partners, and those within the local community to develop services to meet the mental health needs of the community.
  • Employing a wide range of skills, building on and developing their expertise and actively seeking advice and collaboration with issues which may not be readily within their sphere of practice. The requirements for this post are to remain within the guidelines for professional practice inclusive of HCPC/NMC requirements or equivalent professional body.
  • Undertaking comprehensive person-centred psychosocial assessments (conversations), risk assessments and formulation of presenting mental health needs.
  • Building  therapeutic relationships, develop a plan of care with that person and assist them through their difficult time by empowering and enabling them to take control over their health.
  • Supporting and motivating individuals to improve their mental health by helping them develop self-care skills, build resilience, and better utilise community resources. A key part of this developing role will be to make and maintain relationships with community partners including statutory organisations, Voluntary, Community and Social Enterprise (VCSE) organisations and the local communities who we serve.
  • Hearing and responding to the voice of lived experience is a key element for developing this service.

As an employer and sponsor licence holder, to be able to provide sponsorship to overseas nationals via the Skilled Worker route we must ensure that we adhere to the sponsorship requirements set by UK Visa’s and Immigration. Please note that as the current salary threshold is £25,000 it is unlikely we will be able to offer sponsorship for this post as without 2 years previous NHS Service at Band 3 you will commence at the bottom of the pay band which currently does not meet the minimum salary requirement for sponsorship (bank and agency service is excluded). This would apply to anyone who has current time limited right to work in the UK and would require sponsorship in the near future commencing at the bottom of the band.
